NOTE: If an expired military ID card is presented at the security gate as you enter the building, the security guard WILL confiscate it.

Documentation

Proofs of Identity are mandatory
-
Two (2) valid, unexpired forms of identification must be presented to receive any ID card. For a list of authorized proofs of identity, please click here.

All documents must be original. Certified and/or notarized copies of the originals are also accepted.
-
All court documentation must be state or county certified stating that it has been filed and recorded in the courthouse.
-
When a Social Security (SS) number is required, you may use a SS Card or other official documentation showing the SS number (i.e. letter from SS Office with SS number or Military ID).
-
Driver’s Licenses must have an actual date of expiration.

Common Access Card (CAC)
Renewing an Expired CAC

CAC may be renewed up to 90 days in advance of expiration
Lost or Stolen ID Cards

Police report, Sworn Statement, or Counseling Statement (this document will be scanned and saved into DEERS)

Must present two (2) valid forms of identification. (See Proofs of Identity above)
Military Members

Personnel Status must be in DEERS
-
Official documentation must be presented for any DEERS updates or changes (i.e. Promotion Orders, Active Duty Orders over 30 days, Name Change Memorandum, Extensions, Enlistment Contract, Commissioning Oath, DD Form 214, Separation Orders, etc)
DoD Contractors

All data must be previously input through Contractor Verification System (CVS).

If going overseas, a Letter of Authorization (LOA) must be presented reflecting this information

DOD Civilians

In all other locations, a request for recognition must be approved by the appropriate Assistant Secretary of the Military Department concerned or an appropriate official to whom he or she has delegated approval authority.

Note: If the date of birth is not documented in the Placement Agency letter, a child’s birth certificate is required.

If card is lost or stolen, reissue ID card to original expiration date. No new dependency determination is required.

In the case of deceased sponsors, the dependency determination currently in effect or accomplished at the time of the member’s death may be used for subsequent renewals.

Letter from the Social Security Administration indicating that the child is not eligible for Medicare, Part A

A statement that the child is unmarried

NOTE: Temporary Incapacitation - Require a current medical sufficiency statement or physician's statement from a Uniformed Service Medical Treatment Facility and a current dependency determination when renewing an ID card for a child whose incapacitation is temporary.
Full-Time Students (21-23 years old) – Attending accredited institution

For graduate students, a letter of acceptance of enrollment signed by an authorized officer of the college or university is required. NOTE: If student falls below full-time status, the ID card must be turned in to a local ID card facility. Any medical costs incurred will be the responsibility of the Sponsor.
Students attending two institutions less than full-time may not combine courses from both institutions to meet full-time student status
Father, mother, father-in-law, mother-in-law, stepparent, parent-by-adoption

The sponsor's or spouse's birth certificate reflecting parent’s name (as applicable)(if not enrolled in DEERS).
