Emergency State Active Duty Injuries 

When a soldier or airman is injured, have them go to the nearest civilian facility for medical treatment. Advise them to tell the treatment facility they are a "state employee" and the injury is work related.

Note: all treatment bills should be mailed to: the military department, attn: state personnel programs # 27, P.O. box 269101, Sacramento, ca 95826-9101. Compensation for medical bills will come from the state compensation insurance fund (SCIF) who is our insurer.

Within 24 hours of the injury, complete all information including mission number and signatures on the SCIF 3067, SCIF 3301, CAL ARNG form 190-40r and a sworn statement outlining the circumstances of injury. Fax them along with copies of treatment records to (916) 854-3647. Forward the completed and signed originals to: military department, attn: state personnel programs # 27, P.O. box 269101, Sacramento, ca 95826-9101.

Timely and accurate completion of all required forms are essential. Be advised when a soldier is injured and obtains medical treatment, they are responsible for the bills if the above forms are not completed and received by this office.
