Emergency State Active Duty 

BEFORE an emergency occurs:

Check with your unit readiness NCO to ensure you have the proper equipment and trainng for state active duty

DURING an emergency:

All soldiers and airmen called to Emergency State Active Duty (ESAD) must fill out an OTAG Form 14 to cover all days of duty

For all fire-specific missions, a wildfire code must also be included on the Form 14

Some common discrepancies on the Form 14 that may delay your pay:

 Problem  Solution
 Failing to submit a Form 14 Be sure to get the forms transmitted to state personnel by Tuesday of each week of emergency duty
 Duplicate Form 14's Submit Form 14's only once and keep documentation of when it was sent (ex. FedEx tracking #, fax transmittal receipt)
Form 14's that overlap in duty day claims Keep a copy for your records and make sure the same day is not submitted for pay on multiple form 14's
Missing Mission Number (box 10) Do not submit Form 14's without a mission number. They cannot be processed for pay.
Fire missions that are missing fire codes (box 12) Fire missions have a fixed rate of pay for each day of duty and for each type of job performed. Without this code, the claim cannot be processed.
Missing duty Month or Year (box 13) Without the duty Month or Year, the Form 14 cannot be processed
No soldier signature (box 17) The soldier who is making a claim for pay must sign the form
No certification signature (box 18) Every form must be certified by the appropriate authority of the duty that was performed
Incorrect mailing address (box 19)
Sometimes Form 14's are generated by unit admin NCO's using incorect or outdated information from various sources. Be sure the soldier themselves verify the address where the check is to be mailed.
